Community Social Prescribing
What areas do you cover and who do you work with?
We can work with anyone over the age of 18, and are funded to support people who are registered at GP surgeries within what used to be known as the ‘Canterbury and Coastal CCG’ (before it became ‘Kent and Medway ICB’). That includes Canterbury, Whitstable, Herne Bay, and Faversham. We are a friendly bunch, so if you live outside these areas and need support, you can still contact us, and we will try to connect you up with the best service to help which is local to you.
How long do you work with clients?
Sometimes we have a very brief interaction with our social prescribing clients if they just need connecting to one thing and don’t require a follow up. We can work with a client up to 12 sessions (6 weeks from the initial consultation). We will work with clients for a longer period depending on individual needs and our capacity to do so. There is also no limit to the number of times you can refer yourself, or someone else, in to us: sometimes things will come to a close and a while later, you need some more input: that’s human, and totally fine with us! We’re here to help when you are ready.
Can you come and visit me?
We can conduct home visits, but we do this only when there is a specific need or requirement of the client (i.e housebound). We are happy to meet within a safe and comfortable community setting, or via phone, email or video call.
