Children & Young Persons Social Prescribing Herne Bay
What area do you work in and who do you work with?
We can work with anyone the between the ages of 0-18 and their families/guardians. We take a ‘whole family approach’, and are funded to support people who are registered at GP surgeries within the Herne Bay PCN. These are patients registered with Herne & Broomfield, Park Surgery, St Annes Surgery, Beltinge & Reculver Surgery & Hersden Surgery. If your child attends one of the schools we work with n Herne Bay but isn’t registered with these surgeries, we will still support them dependant on our capacity.
Can you come and visit me?
We can conduct home visits, but we do this only when there is a specific need or requirement of the client. We are happy to meet within our school based clinics, Gp surgeries of Herne Bay, a safe and comfortable community setting, or via phone, email or video call.

How do we access the service?
There are many routes into our CYP service: You can self refer, be referred by your GP Surgery, and be referred by other professional services with your consent. We also work within local Primary schools in Herne Bay, holding open clinics where families can find out more about the service and book in directly to see a social prescriber and discuss ‘next steps’ to see how best we can support you. We also run clinics with pre-booked appointments in a variety of venues such as the Herne Bay Family Hub.

How does it work?
The client (and dependant on age, guardian) and the social prescriber will arrange a time to hold a consultation; there is no time pressure with this appointment. During this consultation, you can discuss any challenges you may be facing and what you would like support with. From this meeting, the social prescriber and client will devise a plan of support. This may be providing signposting, assisting with referrals, or a longer term plan involving supporting with attendance to groups or meetings, or referring onwards to other services and ‘coaching’ you through the journey. Usually, we will work with clients anywhere from 2 weeks to a 12 week period, (6-8 sessions of 1-1 support) dependant on need. We will talk about what you want to achieve and using supportive planning will break these goals into small, more manageable steps. We work using a holistic ‘whole family’ approach, so we can support the child/young person and their parent/guardian simultaneously.
