What is Social Prescribing?

Our Social Prescribers support people to make positive changes in their lives and within their communities by linking them to voluntary and community groups and other non-clinical services.

The benefits of this approach include helping people to:

• Feel more involved in the community
• Meet new people
• Improve health and wellbeing

When you refer someone into our Social Prescribing service, they will get an initial call from to introduce and explain our service, and book a consultation with a social prescriber.

The social prescriber will then have a friendly discussion with them to get a holistic view of what’s going on in their life, what challenges they are currently facing and what might be most helpful to connect them to.

There’s no one size fits all solution, because everyone is different – so it’s led by you and what you feel will help, not what someone else thinks you ‘should’ be doing. Here are some of the areas we can offer support regarding:

Housing, Benefits, carer support, social isolation, bereavement, activities, volunteering, counselling, wellbeing and more!

 We are not:

A 24 hour service

An immediate response or crisis service

Able to support with acute medical concerns

Able to fund peoples care or service

Able to directly support with house clearance or decluttering.

Able to support with decorating, refurbishment or moving furniture.

 

We are not a crisis service.

Our office hours are 8am to 4pm, Monday to Friday.
